Create a Question
On the side bar, select 'Questions and Forms':
This will take you to your question bank. From here, you may add new questions by the 'Create Question' button.
From here, fill out the question with Title, Question Type and any additional options.
The Question Types are:
Text Response
Multiple Select
Single Select
Rating Scale
Each for each question, the following can be customized:
Title and description
Question type:
Text Response
Single select
Multiple select
Rating scale
Optional settings:
“Not applicable” option
Additional comments field
Mark question as sensitive (responses to sensitive questions are only visible to admins by default)
For rating scale questions, you can:
Define the scale range
Add custom labels for each value
Edit, Delete, or Duplicate Questions
To edit, delete, or duplicate existing questions, click the three dots on the right:
When you duplicate a question, the new copy includes all of the original question's configuration, including:
Title (automatically prefixed with "Copy of")
Description and any rich text formatting
Question type
Answer format settings, such as scale labels and single- or multi-select options
Additional settings, including:
Allow N/A responses
Additional comments
Sensitive response settings
The cloned question is created as a completely separate question. It does not inherit any submission data, reporting data, or usage history from the original question, and it is not connected to any forms or review cycles where the original question is currently being used.
Create a Form
Navigate to the 'Forms' tab, and select 'Create Form':
From here, add in the title and description then click Create Form:
Once created, this will open the form-builder modal:
You may add questions by clicking either of these buttons:
Click Add Questions to open a modal where you can search and sort through available questions. Use the checkboxes to select the questions you'd like to add.
To remove a question or make it required, click the three-dot menu next to the question. To reorder questions, use the six-dot icon to drag and drop them into your preferred order.
Once the questions are added, you can preview on the right-hand side how the form will appear:
On the upper right corner, you can select the 3 dot menu to edit, duplicate, or delete the form. You can also click 'Save Form' once finished.
Once the questions are saved in a form, they can be selected from the evaluations form list in your cycle settings:
You can also manage questions and forms from within your cycle setting by clicking 'Manage Evaluation Forms' which will take you to the 'Question and Forms' tab shown above.
Important Note: Any changes made to questions or forms will not update in active cycles. Please ensure question details and forms are accurate before inputting them into a cycle.
Edit, Delete, or Duplicate Forms
To edit, delete, or duplicate a form you may click the three dots and select the option you'd like:
Duplicating a form creates a new form using the original as a starting point, copying over all of its existing information. From the duplicating modal, you can make any necessary edits to customize the new form before saving it.
The duplicated form is fully independent - edits to the copy have no effect on the original or any active cycles using it.
Note: Duplicating is the same functionality that was previously called Cloning.
